1. Creating the form
In order to create an evaluation form first go to >Training > Questionnaires. There, choose to to add a questionnaire.
On the next screen you can choose a name (and translations, if needed) and a type. The type governs where the form can be used. "Training expectations" is used for forms filled out before the training. "Default evaluation" and "Effectiveness evaluation" are used after the training is completed, with the first typically used right after the training and the latter further into the future.
2. Adding items to the form
Next, go to the "Items" tab. Here you can add items to create the actual form. You can have the following options:
- Title: This shows the text in a slightly larger font to be used as a title for a section of the form.
- Text: Text entered here shows up on the form. Typically used to give additional information about the form.
- Q&A text: Below the text entered here, a free-form text field is shown on the form, allowing the respondents to answer the question in their own words.
- Q&A choice: Creates a question on the form where the respondents can choose from a pre-defined list. A horizontal lay-out gives a slider that can be used to indicate the answer, a vertical layout gives the possible answers as radio buttons.
- Q&A choice & text: Combines the previous two, allowing the respondents to both choose an answer from a list and enter an answer themselves in a free-form text field.
- General rating: Allows the respondents to give a rating, either in the form of stars or through a sliding scale. The horizontal scale starts at a null point, whereas the vertical scale starts at the first pre-defined option.
- New page: This item will put all the following items on a new page of the form. Useful for breaking down longer forms.
3. Adding pre-defined options
Some items require to have pre-defined options to function properly. In order to add these options, choose to edit the item. Then, click "Add choice."
Each choice requires a value, which is not shown on the form but is used for later reporting. The text entered under "Choice" is what is shown for each relevant language.
Confirm the creation of the choice item by clicking "Insert" at the left.
Attention: After adding all choice items, confirm attaching them to the questionnaire item by clicking the "Save" button.
4. Adding the form to a training type
In order to add the form you created to a training type, first go to Training > Training types
On this page, click "Configure" under "Evaluation settings."
Scroll down and click edit. Now you can choose which evaluation form you wish to use, when it should be available, and for how long.
If you want the form to be active immediately after the training was completed, you can enter value "0" in the field "Active after (# days)".
